健康診断報告2014年1月19日

昨日は毎月1回の健康診断でした。

今回は日本の薬剤師のマリちゃんと友人のリョータ君のヘルプがありました!
ありがと~

到着したときは、「あ、今日はちょっと少なめかな~」っていう感じだったんですが・・・
いえいえ・・・とんでもない。

あれよあれよというまに・・・30人が40人に・・そして60人にまで膨れ上がりました。

ここ最近、カンボジアでは肌寒い日が続いていて、めずらしく私もカーディガンを羽織ってました。
なので、鼻かぜや咳といった人が多かったように思います。

今回は結核の可能性があるのに、病院が信用できずに点滴を途中で引き抜いてまできてくれたひとがいました。
でも、血痰もでてるし、3ヶ月も咳が続いていて、脇腹も痛いというので公的な病院へ行って、結核の無料診療を受けるように説明しました。

野党の活躍で、ここ最近、隠れてた汚職が表面化されてきたのですが、どうも保健センターにも汚職があったようです。
本来ならば、結核や出産などは無料なはずなのですが、そこをお金を取り立てるそうです。

公的な保険センターで働く職員も、みんな副業として「手伝っている」人がほとんどで、専門職ではありません。その人たちが数週間のトレーニングを受けて、保健省から支持されたマニュアル通りに問診をして薬を渡しています。
まるで・・・チャート式のように。

今回は保健センターのなかにジアゼパムもみつけました。

この怖いこと。

汚職率の高い保健省・・・・保健センターを管理するべきの職員の意識は本当にどうなっているんでしょうか・・・。

もうひとりC型肝炎の人がマスクをして、家族に染らないかと心配で来た。
C型は空気感染はせず、血液感染をするので、けがをした時や出血をした時だけ気を付けるように説明をしました。
さらに、糖尿病と診断されたので、血糖降下剤ももらっていましたが、肝炎患者は血糖が上がりやすくなるので、簡単に糖尿病と診断ができないのですが。
薬も本来なら慎重にすべきで、薬を処方する前にまずは食事療法をしていかないといけないはずなのですが、そこがどうやらごっそり抜けているようです。

こちらで勝手に薬を減らすことはできないので、微熱やだるいことがあったらすぐ薬を中止してすぐに病院へ行くように説明をしました。

When we arrived there, we felt that there were not so many patients, but! no,no,no!
On the way we were doing, increase patients step by step...and finally, the number of patients was 63!

This time, we faced the two patients.
One of them, lady got cough and diagnosed lung disease in Cambodian clinic.
But she didn't want to believe and came to us.
After my interview, I made sure that there is strongly possibility of Tuberclosis because of bloody phlegm and pain in both side of body.

I recommended to go to public hospital to be checked.

Another was got C-type hepatitis and worry about infuluence to family.
I explained that this virus was influenced with blood, so that normally, no need worry.
And when he injured or bleeded, please pay attention.
He diagnosed diebetis, also.
But the level of glucose rise up easier than usual in case hepatitis.
His doctor gave him many diabetis medicien, so that I was really worry.
I advised to him that it was important to check his body condition, and if he feel tired and slight fever, please stop medicine and go to clinic.

I noticed that there was Diazepam bottle in healthcare center.
This medicine is 'control medicin' in many countries.
But in Cambodia healthcare center, this medicine is put on desk as same as other medicines...
Wow~
As pharmacist, I can not accept this situation.
MoH need to control and manage the high risk medicine...

I relized that there are many thing we need to support to medicine affairs matter as Japanese pharmacist.
